AGREEMENT TERMS AND CONDITIONS
transaction.  The monthly membership fee is specified above.  For purposes of identification and billing, you agree to provide us with current, accurate, complete and updated information including your name, address, telephone number and applicable payment data. You agree to notify us promptly of any changes in your membership data.  Failure to do so will result in a loss of any unused services
.  Your membership status must be active to redeem membership services and benefits.  As long as your monthly membership payment is made timely, you will be considered an active member in good standing.  If your payment account does not contain sufficient funds to complete the transaction, or your payment account or credit card does not otherwise permit the transaction to be executed, you will be charged a thirty-five dollar ($35) insufficient funds fee per incident and may be subject to revocation of membership as well as loss of all unused services.  If your membership account becomes delinquent and is not properly canceled, the account will be referred to collections, and you agree to pay all reasonable collections, agency fees, and legal costs incurred.
_________This membership and services purchased under this membership are non-transferable and have no cash value.  Unused services
maybe carried over during the term of this agreement but must be used with in twelve (12) months from purchase date.  I understand my membership is auto-renewed. Following the initial term, your membership will automatically continue on a month-to-month basis at the predetermined rate until your membership is canceled or terminated as provided in this agreement.  Upon termination or cancellation of membership, all unused services will expire.
*Services provided by Christina Mitchell and  Kari Alewine of Just Waxed may be excluded from this offer.
**Membership discounts cannot be combined and may not be used with any other offer.
CANCELLATION OF MEMBERSHIP
______Cancellation of membership during the initial term will be allowed upon your death, physician certification of permanent physical disability or inability to receive services, or upon proof of relocation of residence greater than sixty (60) miles from spa location.  Membership may be canceled within first fourteen (14) days of the initial term. However, a fee of no more than twenty-five dollars ($25) per service will be charged for any services received before the termination date.
_______Cancellation of membership outside of the accordance of this agreement will result in a fee of fifty percent (50%) for each remaining month left in the contract at the time of cancellation effective date, and all unused services will be forfeited.
_______Cancellation of membership after the initial term, during month-to-month membership, requires thirty (30) days

______Radiant Health Day Spa reserves the right to revoke, deny re-enrollment or terminate membership at any time for misconduct or lewd behavior on our premises or if you have an unsatisfactory payment history.  You will be responsible for payment in full upon revocation of membership.

________I acknowledge it is my choice to receive massage therapy. I am aware of the benefits and risks of massage and give my consent for massage. I understand that there is no implied or stated guarantee of success of effectiveness of individual techniques or series of appointments. I acknowledge that massage therapy is not a substitute for medical care, medical examination or diagnosis. I have disclosed all medical conditions that I am aware of and will inform my practitioner of any changes in my health status.  I agree to communicate immediately with my practitioner anytime I feel my well-being is being compromised. I hereby waive and release Radiant Health Day Spa and assigns from any and all liability, past, present, and future relating to services, massage therapy and bodywork provided by anyone from Radiant Health Day Spa.
